Your body may be sending you signals. OCD is making it impossible to know which ones to trust.

Health OCD and health anxiety sit at one of the most difficult intersections in mental health because the fear is not irrational on its face. Bodies do get sick. Medical events do happen. The problem is not that you are paying attention to your health. The problem is that OCD has taken that attention and turned it into a loop that is consuming your life and, paradoxically, making your symptoms worse.

If you have a real medical condition alongside health OCD, you are not imagining things and you are not being dramatic. Health OCD is extremely common in people who live with chronic illness, autoimmune conditions, and unexplained symptoms. The anxiety and the medical reality can coexist, and both deserve to be treated.

How Health OCD shows up

It might look like Googling symptoms for hours. Opening Reddit threads about your condition at midnight. Entering your symptoms into AI and asking for a diagnosis. Checking your Apple Watch, Oura Ring, or Whoop data compulsively throughout the day, looking for evidence that something is wrong or reassurance that everything is fine. None of it ever resolves the anxiety. That is the loop.

It shows up as fear that you cannot trust your medical team, that you have not been heard, that something is being missed. Sometimes you do need a different medical team and other times you do not and that is part of what is hard about this presentation. However, if you have compulsively gone to specialists, that is when it is clearly OCD and a new medical team is not the answer.

Some of my clients spent years without a diagnosis before finally receiving one, an autoimmune condition, a chronic illness, something that was real all along. That history makes it genuinely difficult to know when a symptom is worth worrying about vs. when it is health OCD driving the urge to seek more answers. OCD exploits that uncertainty completely.

Health OCD shows up as checking your body, your devices, your search results, and then more checking when the first check did not resolve anything. Avoiding activities you love because of fear about what they might do to your body. Restructuring your entire routine around your symptoms in ways that are shrinking your life rather than protecting your health.

AI is making health OCD significantly worse and it is worth naming directly. Entering your symptoms into ChatGPT or another AI tool and asking for a diagnosis is a compulsion. The AI will give you an answer. The answer will not resolve the anxiety. It will give you something new to research, something new to fear, and send you back to Google. The loop gets tighter.

The AI problem

Why standard therapy often misses it

Health OCD sits at the overlap between OCD and health anxiety, and the treatment for both involves ERP. Standard talk therapy can make it worse by providing reassurance that temporarily relieves anxiety and feeds the loop. If a therapist has been helping you process your health fears without doing exposure work, you have probably noticed that the fears keep coming back.

The other reason it gets missed is the real medical component. When a client has an actual diagnosis or ongoing symptoms, it can feel irresponsible to challenge the checking and reassurance seeking. But what we know is that anxiety around somatic and health symptoms makes those symptoms louder. The compulsions amplify the signal. Turning down the volume on the anxiety is not ignoring your health, it is actually better health data.

What The Work Looks Like

We start by mapping your specific health OCD loop. What triggers the checking, what the reassurance seeking is giving you temporarily, and what response prevention looks like for you specifically. For clients with real medical conditions we work with your actual health reality while building the skills to sit with uncertainty about what your symptoms mean.

Exposures for health OCD are often somatic: sitting with physical sensations without checking, decreasing Apple Watch or Oura Ring data reviews, resisting the Google search, tolerating uncertainty about a symptom without seeking reassurance. Exposures can also include getting you back to valued living and activities you have been avoiding.

The goal is not to stop caring about your health. It is to live fully while experiencing symptoms rather than organizing your entire life around them.

That is what life outside of health OCD looks like. Not the absence of symptoms or fear. The ability to make decisions from your values rather than your anxiety, and to show up for your life while your body does what bodies do.

Having these thoughts or experiences occasionally is normal. What distinguishes OCD from occasional anxious thoughts is that the thoughts are persistent, feel completely out of character, and cause significant distress and disruption to daily life.

If you are unsure whether what you are experiencing is OCD, working with a psychologist who specializes in OCD and anxiety treatment is the best way to get clarity on whether you meet criteria for OCD or another anxiety presentation.
